Case Manager

Description

The Boys & Girls Clubs of Greater Tarrant County is currently accepting applications for Comin Up Case Manager. The Comin’ Up Case Manager is, under direct supervision of the CU Site Director responsible for case management, participation, Comin Up programming, administrative duties, facility and child safety.

 

Duties include but not limited to: provide holistic and comprehensive case management services to members at assigned site(s); accountable for promoting site participation; assigned programming and activities; participate on multi-disciplinary team; document all services and contacts, and administrative compliance. Services are provided on site during program hours. May be required to provide support services virtually. May be required to assist with community outreach. There is potential for exposure to germs and illnesses. Employees are to follow all safety procedures and protocols.

 

Work sites are located in low income areas, challenging neighborhoods, and growth neighborhoods. The noise level in the work environment is usually loud and at times moderate or very loud. Position requires working with at risk and gang involved youth.

The Comin' Up Gang Intervention Program services at-risk and gang-invovled youth between the ages of 13-24 years in Fort Worth communities. Comin Up Program provides needs-based services and activities in efforts to reduce gange violence and influence positive change.

 

Requirements

Education and Work Experience Requirements:

• Bachelor’s Degree from an accredited college or university required, Master’s Degree preferred. Preferred majors and/or degrees include Social Work and Counseling. Licensed social worker or counselor preferred
• Minimum of one year experience working with at risk youth and/or gang involved youth
• Minimum of 6 months experience with case management

 

Minimum Qualifications Requirements:

• Ability to work regular program hours which includes day, evening and night hours with periodic weekends (required evening programming hours are 7:00pm – 11:00pm Monday – Friday; required day time hours will vary based on program need)
• Ability to assist youth to progress and achieve their individual goals
• Ability to establish rapport and build relationships
• Ability to identify needs and find solutions
• Ability to work with at-risk youth
• Knowledge of case management techniques and/or processes
• Knowledge of conflict resolution or mediation techniques
• Knowledge of problems and barriers at-risk youth face
• Knowledge of technology and software such as tablets, social media, cloud based tools, virtual meeting tools
• Must be a team player, willing to collaborate and make team decisions
• Must be able to obtain CPR and First Aid certification
• Must be able to pass any and all required background screens and drug screens at any time
• Position requires driving personal vehicle and company vehicle. Candidate must have satisfactory driving record, valid driver’s license, reliable transportation, and liability insurance for operation of a motor vehicle relevant to carrying out duties and to be covered under company insurance the requirement is for the employee to be a minimum of age 21

 

Pay & Benefits

Pay: $19.51 per hour
