POSITION SUMMARY The hospice aide is a member of the interdisciplinary group who works under supervision to provide personal hygiene care to hospice patients, following the patient’s Plan of Care.
ESSENTIAL FUNCTIONS
Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.
Meet safety needs of the patient using equipment safely and properly (foot stools, side rails, O2, etc.)
Deliver routine medical supplies as needed to patients.
Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
Observe and report any patient condition changes to the RN Case Manager.
Provide proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence.
Demonstrate respect and understanding of the philosophy, mission, and service standards of CHOT for providing excellent customer service.
Successfully complete 12 hours of in-service training annually.
Assist the patient with transfers as needed.
Exhibit respect of patient and family/caregiver environment and patient’s personal needs.
Provide compassionate companionship to patients while delivering services.
POSITION QUALIFICATIONS Requires a HS degree or GED, and proof of State Certification 1 year of direct patient care experience required
