POSITION SUMMARY
The Case Manager meets with patients and families to explain the scope of hospice services, coordinates admission of patient and the ongoing plan of care.
ESSENTIAL FUNCTIONS
- Provide professional nursing care by utilizing all elements of nursing process
- Assess and evaluate patient’s status by writing and initiating plan of care. Regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary
- Develop a care plan that establishes goals, based on nursing diagnosis and incorporates hospice nursing actions. Include the patient and the family in the planning process
- Administer medications and treatments as prescribed by the physician
- Provide healthcare instruction to the patient as appropriate per assessment and plan
- Act as Case Manager when assigned by clinical Supervisor and assume responsibility to coordinate patient care for assigned caseload
- Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate
- Communicate with the physician regarding the patient’s needs and reports changes in the patient’s condition, obtains/receives physicians’ orders as needed
- Communicate with community health related persons to coordinate the care plan
- Provide and maintain a safe environment for the patient
- Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the needs of the patient and family/caregiver
- Attend interdisciplinary group meetings
- Participate in on-call rotation; may assist with admits
- Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis
- Supervise LVN and CNA care plan and delegate responsibilities when required.
POSITION QUALIFICATIONS/REQUIREMENTS
Registered Nurse in the state of Texas
Hold a valid driver’s license and have a dependable means of transportation
1 year of nursing experience strongly preferred
2+ years of experience in hospice case management strongly preferred
Proficient skill using a computer is required, Microsoft office suite experience preferred.
