RN Case Manager

POSITION SUMMARY
The Case Manager meets with patients and families to explain the scope of hospice services, coordinates admission of patient and the ongoing plan of care.
 
ESSENTIAL FUNCTIONS
 

  1. Provide professional nursing care by utilizing all elements of nursing process
  2. Assess and evaluate patient’s status by writing and initiating plan of care. Regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary
  3. Develop a care plan that establishes goals, based on nursing diagnosis and incorporates hospice nursing actions.  Include the patient and the family in the planning process
  4. Administer medications and treatments as prescribed by the physician
  5. Provide healthcare instruction to the patient as appropriate per assessment and plan
  6. Act as Case Manager when assigned by clinical Supervisor and assume responsibility to coordinate patient care for assigned caseload
  7. Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given.  Record pain/symptom management changes/outcomes as appropriate
  8. Communicate with the physician regarding the patient’s needs and reports changes in the patient’s condition, obtains/receives physicians’ orders as needed
  9. Communicate with community health related persons to coordinate the care plan
  10. Provide and maintain a safe environment for the patient
  11. Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the needs of the patient and family/caregiver
  12. Attend interdisciplinary group meetings
  13. Participate in on-call rotation; may assist with admits
  14. Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis
  15. Supervise LVN and CNA care plan and delegate responsibilities when required.
POSITION QUALIFICATIONS/REQUIREMENTS
Registered Nurse in the state of Texas
Hold a valid driver’s license and have a dependable means of transportation
1 year of nursing experience strongly preferred
2+ years of experience in hospice case management strongly preferred
Proficient skill using a computer is required, Microsoft office suite experience preferred.
